SERVICES WE PROVIDE

Non-Skilled Services (HHA/CAN)
HHA/CNA to provide personal care services including assistance with bathing, grooming, dressing , assist with reminder of oral medications. Light housekeeping, companion, respite, shores, grocery shopping, medication pick up. All this services aide in promoting independence, keep vital routines in place and helps patients age in place with dignity.
 
Skilled Nursing Services
RN/LPN services, from evaluation, monitoring  management of pre-existing condition such as diabetes  and hypertension,, education on new medications and disease process, wound care, IV therapy,, tracheostomy care, and more. Stay Home with Hospital like services. (24/7Skilled nursing assistance available )
 
Rehab Services (PT/OT)
Physical and Occupational Therapy at home anywhere from evaluation, treatment , education  with the main  goal to improve functionality, independence, and decrease pain and  risk for falls and injuries.
 
Social Services
Our Social Services team will evaluate needs and assist proactively with Community Referrals, applications for available and eligible benefits such as Medicaid, LTC, Meals on Wheels and more!
